No one likes paperwork! At The Fitness Lab, we value your time. In order to best serve you, we have streamlined our intake paperwork as much as possible. We recommend filling out your paperwork before your evaluation to maximize your time with your trainer and/ or physical therapist. Feel free to either download, electronically fill out, and securely email the forms back to us at Fit@fitnesslabco.com or print them out and bring them into us on the day of your evaluation. If you are unable to print the forms, please arrive 15 minutes prior to your scheduled appointment time.
